Last year, we supported thousands of disabled people and people with long-term health conditions to live independently, stay connected, and access the support they need.
We responded to over 5,800 advice enquiries, delivered more than 27,000 hours of care in people’s homes, and helped hundreds of people return home safely from hospital. Through our community work, we also connected with over 4,700 people, reducing isolation and making sure lived experience shapes the services that affect their lives.
