About the Service
Our Hospital Discharge Support service supports people to enhance their independence and day to day lives on their return home from hospital.
Our Hospital Discharge Support service concentrate on practical help that maintains independence and wellbeing. We work alongside University Hospitals Sussex discharge teams to do this.
We arrange grants to pay for works to make homes suitable to return to, allowing people to be more independent and recover at home.
Providing support in this way reduces the likelihood of readmission to hospital.
The service is funded by NHS Sussex Integrated Care Board (ICB) and Brighton and Hove City Council’s Disabled Facilities Grant.
