About the Service

Our Hospital Discharge Support service (previously called Link Back) is funded by NHS Brighton and Hove Clinical Commissioning Group (CCG) to link people to community, voluntary and private sector services that can enhance their independence and day to day lives on their return home from hospital.

Our Hospital Discharge Support team provide assistance to vulnerable patients to safely return home after a stay in hospital, and to help reduce the likelihood of readmission. We work alongside University Hospitals Sussex discharge teams to do this.

We combine access to hospital discharge funding so vulnerable patients can be safely discharged from hospital with ongoing support from a link worker. This helps people to become more independent and recover at home.

Providing support in this way reduces the likelihood of readmission to hospital.

The service is funded by NHS Brighton and Hove Clinical Commissioning Group (CCG) and Brighton and Hove City Council’s Disabled Facilities Grant.

Possability People both support with immediate practical tasks to get individuals home – and follow up with support to help find services or provide advice to promote patients to stay at home as independently as they can.
