Early Response Worker

Fixed term contract until March 2022, with possibility of extension

We are looking for a creative, talented and hardworking individual to join our Link Back Service which supports discharge at University Hospitals Sussex (UHS) by helping older and disabled patients access the support they need to facilitate a safe discharge home and their ongoing independence, health and wellbeing.

Working within our existing Early Response team you will support eligible patients to access Brighton and Hove City Council’s Hospital Discharge Grant (HDG). The HDG provides financial assistance to carry out housing interventions and provision that will avoid delayed transfers of care and help avoid unnecessary readmission to hospital.

Full time post:           35 hours per week

Salary:                        £25,481.00 per annum
